New Mexico crime authorities may not yet know who to blame Halyna Hutchins‘ dead – but a state agency puts the hammer down… pointing to the production as a whole.
The Office of Occupational Health and Safety – which deals with this sort of thing throughout New Mexico – imposed a hefty fine of $139,793 this week after discovering that Rust Movie Productions (the production behind Alec Baldwin‘s flick) largely screwed the pooch when it came to on-set gun safety.
The government entity claims that RMP failed on every level in maintaining and enforcing industry-standard protocols for the use of firearms in film – and as a result, someone died.
They point to two different misfires that had occurred during filming prior to the Hutchins incident, which the bureau says were largely unresolved by the official security point among the crew…in this case, they say it was the assistant director Dave Halls.
The report notes that several crew members expressed concern over these misfires – among other weapon-related concerns – but it would appear that no one really did anything to fix it or even remedy it…via training or discipline, etc.
They also say that for the fatal shooting, Halls again broke protocol by handling the gun and giving it to Baldwin…without consulting a specialist during or after loading the gun. Speaking of which, they draw a damning conclusion about live ammunition – suggesting that production heads had no system in place to ensure that real bullets weren’t heading into the set… which they say is a clear violation of basic procedure for this sort of thing.
A Bureau representative said, “Management had multiple opportunities to take corrective action and chose not to. As a result of these failures, the Director Joel Souza and cinematographer Halyna Hutchins were seriously injured. Halyna Hutchins succumbed to her injuries.”
Again, OHSB says the production company as a whole appears to be responsible here — but they’re doing their best to name Halls in their remarks and conclusions. Their investigation was thorough… with 1,500 staff hours dedicated to sorting it all out in stride.

the criminal investigation is still ongoing, and of course… lawsuits have been filed against Baldwin, the production company and others. It’s unclear how this report might affect things.
